What is the Referral Process?

All youth are referred to The Children’s Home of Wheeling, Inc. through the West Virginia Department of Health and Human Resources. Referrals are reviewed by the clinical team and evaluated based on several factors including individual needs, admission criteria and bed availability. How do I refer a youth to The Children’s Home of Wheeling, Inc.?

All youth must be referred through the FACTS (Families and Children Tracking System)  system using the Automated Program Referral (APR). 

What information is needed to submit a referral? ​

In addition to the information requested through the APR a WVDHHR Youth Program  Referral Form must be submitted as well as any supporting documentation.
